Employee Struck by Steel Rack Dies

At 9:00 a.m. on June 3, 2008, Employee #1 was in the process of removing 2 rigging clamps from a steel sheet being elevated by a crane about 1 ft above the floor, attempting to place it into a metal rack. Six steel sheets were already stored in the rack. Employee #1 was standing on a 10 ft A frame ladder waiting for the crane operator to set the steel sheet down. The ladder was placed between 2 racks of steel. As the crane operator was lowering the steel sheet down, the bars on the end of the metal rack failed causing Employee #1 to be caught between the 2 racks. Employee #1 was taken to the emergency room where he was pronounced dead.
