Employee Is Crushed by Machine Part and Is Killed

On October 31, 2007, Employee #1 was assisting two other employees clean up around a stacker machine. The employees had received training on not entering into the area under the stacker machine while it was running. Certain sections of the stacker machine had guardrails in place with warning signs. On the South side of the stacker machine, there were no guardrails in place and no warning signs. Employee #1 entered into the area without the other two employee's knowledge and began picking up "sticks" under the horizontal arms of the stacker and between and under the roller the stacked lumber is set on. While Employee #1 was in this area the stacker arms came down and crushed him. Employee #1 was killed.
