Employee Is Killed When Forklift Overturn

On July 10, 2007, an employee was stacking wooden pallets. She was operating a Toyota Forklift to pick up the pallets and stack them 17 feet high. She was then loading them onto a tractor trailer truck. The wooden pallets were located next to several overhead doors. The employee picked-up six wooden pallets in front of one of three doors, Number 12, 13 or 14, and then proceeded to door Number 15, where she had the stack located. The employee was operating at a high rate of speed and as she was turning the forklift to her right she was raising the metal clamps. At this time, the right side of the forklift lifted up off the floor and the forklift fell on its left side. The employee was not utilizing the equipped seat belt, and the employee was thrown from the operator's seat. The forklift's metal cage landed on the employee's head. The employee sustained a broken neck and head trauma. Emergency medical services were called and arrived on site. The employee was pronounced dead at the site.
