Investigation Summary
On April 2, 2007, an employee was operating Raymond Order picker, Serial Number EASI-01-AT27024. It was fully extended to 31 feet 6 inches, pulling orders from the shelves when another operator using a Raymond Turret struck the order picker causing it to tip over and the employee, who was tied off to the machine as required, fell with the machine 31 feet to the floor. The employee was killed.
