At approximately 2:45 p.m. on April 18, 2007, Employee #1 was preparing a load of 4 ft by 8 ft sheets of laminate material on the floor of a warehouse. Approximately 16 sheets of 0.75-in. thick laminate material fell from rack Number E04/05 striking and killing Employee #1.
