Employee Dies in Fall from Ladder

On March 2, 2007, Employee #1 was working from an 8-ft stepladder, trying to disable an Akbar 89 Fire Curtain. The disconnect could not be located because it was not properly marked in the Emergency Service Panel Directory. Therefore, electrical power was disconnected by cutting the electrical supply wires to the curtain. The fire curtain initiated its 10 second delay in closing, and then closed. Employee #1 placed his right foot into the path of the closing curtain, and was struck by the fire curtain, knocking him off of the 8-ft stepladder. He struck his head on floor and was transported to University Medical Center, where he was pronounced dead one hour later.
