Investigation Summary

At approximately 8:44 a.m. on April 3, 2007, the employee, a foreman, working alone, had just started work on a existing sky light screened in porch section of a existing residential home. The porch section measured 24-ft by 12-ft at 1 in 12 pitch with 7-ft-10-in. ground to eave height. The employee had removed a small 4-ft by 6-ft portion of the sky light panel to survey water damage. The main roof structure trusses were wood 4-in. by 6-in. by 12-ft beams on 4-ft feet centers at 1 in 12 pitch. Wood, 2-in. by 4-in., standing on edge were nailed in a cross pattern which made up the rest of the roof structure framing. While standing on the wood, 2-in. by 4-in., framing to survey repairs the 2-in. by 4-in. framing failed under the employee's weight. He fell through that rotted portion of the roof to the concrete floor below and hit the back of his head. The emergency medical services were called and the employee was transported to Huntington Hospital in Huntington West Virginia for treatment. The employee, after 15 days on life support, never regained consciousness and pronounced dead April 17.
