Investigation Summary
On January 31, 2007, Employee #1, a licensed general contractor, was building his own commercial building on his property. He arrived at the worksite and went up onto the roof to check the progress of the roof sheathing work. All of the sheathing had been put down, with the exception of some sheathing at various areas of the roof eaves, and a 48-inch by 48-inch area located at the south end of the ridge of the upper roof section. It appears that while Employee #1 was looking around on the roof, he accidently stepped through the unsheathed opening. He fell approximately 30 feet to the concrete floor and was killed.
