Employee Is Killed When He Falls from Forklift Platform

On May 21, 2007, a coworker, employed by the host employer, elevated Employee #1, employed by the contractor, on a work platform with guardrails using a forklift. The work platform was not secured to the forklift. Employee #1 moved from the center of the work platform towards one side and the platform flipped off of the forks striking Employee #1 in the head. He fell to the ground and died.
