Investigation Summary
On September 9, 2008 Employee #1, a 42 year old male roofer, with Mainstay Business Solutions, was part of a three-man crew on a roofing project. An applicator Employee #2, a hose tender Employee #3, and Employee #1 were tasked to use a windscreen to prevent material drift. The large single story tilt-up type commercial building had a thirty-six inch parapet encompassing the roof which had sixty-one domed type Plexiglas skylights measuring four-feet by eight-feet each. Skylights were masked with paper material and yellow caution tape placed around the periphery. Employee #1 observed the material hose being pulled over a skylight and went to lift it off to prevent damage to the masking paper by the hose. Employee #1 was in a kneeling position next to the skylights long axis, and stretched out to lift the hose off the skylight. As Employee #1 lifted the hose, Employee #3 whipped the hose causing Employee #1 to over extend his body and lose his balance to fall through the skylight to the hard surface floor approximated to be twenty-feet below. Employee #1 was hospitalized and suffered fractures of the right foot and back from the fall.
