Employee Is Injured When Jack Fails

On July 25, 2008, Employee #1, a refrigeration and plumbing technician, was driving a company service half-ton Chevrolet van in Fresno, California. The vehicle had a flat left rear tire, and Employee #1 pulled it to the parking lot of a fast food restaurant, where shade was available. Employee #1 contacted a dispatcher, who authorized Employee #1 to change the tire. Employee #1 called his girlfriend, who was in the area to bring him some water. Employee #1 removed the manufacturers' provided jack, although he could not determine how to remove the spare tire from under the vehicle. Employee #1 raised the vehicle using a jack and crawled under the vehicle to release the spare tire. At that time, the jack failed to support the vehicle and fell on Employee #1. His girlfriend's cries alerted nine employees from two restaurants, who lifted the van, allowing extrication of Employee #1 from under the van. Employee #1's chest was compressed, and he sustained clinical signs of hypoxia. Employee #1 was transported by ambulance to Fresno Community Regional Medical Center for treatment. He was not hospitalized. Upon investigation, it was revealed that the work safe practices and procedures were not followed. The jack's failure to support the vehicle was attributed to the sloped grade of the asphalt parking lot, hot ambient temperature softening asphalt surface, and failure to engage the parking brake, which allowed the vehicle to roll of the jack. In addition, the substitute dispatcher was not informed of the company's policy restricting vehicle repairs to maintenance personnel or authorization for emergency roadside assistance.
