Investigation Summary
At approximately 2:00 p.m. on April 10, 2008, Employee #1, a driver with Sequoia Beverage, was performing his regularly-assigned duties at a client's store. He was hanging a string of promotional pennants from the store ceiling using the merchant's three-legged fruit tree ladder. When the ladder became unstable, Employee #1 fell approximately 7 ft to the floor. He was hospitalized at Sierra View Hospital for unspecified fractures.
