Investigation Summary
At approximately 9:30 p.m. on September 13, 2008, Employee #1 was operating a Moxie; Model Number MT36, 35-ton capacity, articulated dump truck. Employee #1 was hauling road base gravel from a remote stockpile to a staging area about 7.1 miles away, on a semi-public, mountainous and winding gravel road. At the time of the incident Employee #1 had approached a right-hand quick turn that would have taken him around the mountain he was descending. The rig did not make the curve and proceeded straight ahead and over the edge of the mountain. The rig went down the mountainside for approximately 0.25 miles coming to a stop against an embankment in the arroyo below. The force of the collision crushed the operator's cab and Employee #1. Employee #1 was killed.
