Event: 12/13/2007
Employee Is Struck and Injured by Forklift Load

At approximately 12:47 p.m. on December 13, 2007, Employee #1 was conducting his regular duties as a maintenance mechanic for a winery. As he was walking across the company's paved yard between warehouses, he was struck by a forklift (Mitsubishi Model Number FGC25, Serial Number AF82A-51230) that was hauling a pallet of filled and boxed wine bottles. The forklift driver did not see Employee #1 when he walked in front of the forklift, from the left towards the right, as viewed from the driver's perspective. When the forklift driver noticed Employee #1, he applied the brakes and the forklift stopped. Employee #1 was knocked to the ground, and the load's forward momentum caused the pallet to slide partly off of the forks and tip some of the boxes onto Employee #1's elbow. Employee #1 sustained a serious compound fracture injury to his elbow. He was transported to the Fresno Regional Medical Center for treatment. The Sanger Fire Department notified the DOSH Fresno District Office about the accident on December 20, 2007.
