Investigation Summary
At approximately 9:30 a.m. on August 13, 2007, Employee #1 was installing a new shank on a ripper that also had a crumbler bar. As he installed the new shank, he jolted the frame of the ripper and the crumbler bar came down. It pinned him to the ground, fracturing his vertebra and hip. Employee #1 was transported to Kaweah Delta Hospital, where he was hospitalized for four days for observation.
