Investigation Summary
At approximately 3:00 p.m. on July 30, 2007, Employee #1 was working as a mechanic. Employee #1 was replacing a lever bracket in the hub of a Mitsubishi 1000 wind turbine generator. Employee #1 inserted his left middle finger into the shaft opening of the bracket to feel if the two brackets were aligned when the brackets shifted. Employee #1 suffered the amputation of the finger. Employee #1 was treated but was not hospitalized.
