Event: 08/23/2007
Employee Fractures Ribs When Belting Material Falls

At 2:24 p.m. on August 23, 2007, Employee #1 was working in an industrial rubber warehouse. He was using a forklift and was setting up a roll of belting material to be cut on a slitter machine. Employee #1 thought that the material became stuck, and he exited the forklift. Employee #1 pulled on the loose end of the belting, causing the 200-lb. roll of material to come off the forks and hit him in the upper left portion of his back and shoulder. He sustained six broken ribs, and he damaged soft tissue to this area. Employee #1 was held at the hospital for three days for observation only.
