Employee's Hand Is Crushed When Struck by Falling Tine

On February 9, 2007, an employee and a coworker were reversing the rubber wear guard on the bucket of a CAT 924 loader. They were removing the last retaining bolt with a combination wrench, although could not exert enough leverage to loosen the rusted nut. They used a Toyota fork lift equipped with a tine turret to apply pressure to the wrench with the low tine. The tines were rotated ninety degrees to the vertical. The coworker was holding the wrench with right hand resting on the lower tine. The vibration of the forklift or its movements caused the unsecured upper tine to fall, approximately two feet, striking the employee's right hand. The employee was hospitalized as a result of crushing injuries to his hand. Tine carriage mount was equipped with spring loaded detente and gate that failed to prevent movement of the tine or was not engaged. The defect in the tine retaining mechanism was not identified on inspection of the equipment.
