Investigation Summary
At approximately 1:56 p.m. on October 23, 2008, Employee #1 was working on the Catacombs portion of a show set. Employee #1 climbed up onto 12-in. by 12-in. aluminum truss that was approximately 4 ft. above a 4-ft. by 8-ft. wood flooring to move a drape. Employee #1 lost his grip and fell to the floor. Employee #1 sustained a compound fracture of the right heel and a fracture of the right wrist. Employee #1 was hospitalized and treated for his injuries.
