Investigation Summary

Investigation Nr: 201072790
Event: 07/16/2008
Employee gets arm amputated by conveyor belt

At 6:40 AM on July 16, 2008 Employee #1, a scrap metal sorter, was injured. Employee #1's right arm was amputated from the elbow down when it got caught in the sprocket and rollers located at the end of the conveyor belt. The CP Manufacturing Inc conveyor belt is attached to a Harris Waste Management Group, Inc baler. The conveyor belt runs in a loop bringing scrap metal from the ground level up to the baler where it can be formed into a rectangular bale. Part of the conveyor belt is hidden underground in a basement for approximately 50 feet long, 12 feet wide and six feet high. The conveyor belt loops at the end of basement room and moves upward to the surface where the sorters sort the different types of scrap metal. The conveyor belt moves across the surface with the selected scrap metal and then climbs upward to the top of the elevated platform where the scarp metal falls into the top opening of the baler. Employee #1 entered the basement section of the conveyor belt to unjam several pieces of stainless steel scrap metal caught inside the underground section of the conveyor belt. Employee #1 had successfully cleared four pieces of stainless steel scrap metal as he worked inside the basement underneath the conveyor belt. As a check to see if any more pieces of scrap metal are jammed in the underside section of conveyor belt the baler operator (who is located up on an elevated platform above the conveyor) jogs the belt. When the baler operator jogged the conveyor belt employee #1 heard another piece of scrap metal still jammed in the conveyor belt. The baler operator jogged the conveyor belt again to ensure it was working properly and that all the scrap metal pieces were dislodged. It was at this time, that employee #1 got his right arm shirt sleeve caught in the sprocket and rollers at the end of the conveyor belt when he reached in to remove the fifth piece of scrap metal. Employee #1's right arm was amputated between the rollers and the front of the sprocket located at the end of the conveyor belt where it loops upward toward the surface. Employee #1 and his amputated arm were taken by paramedics to the UCSD Hospital. The amputated arm was unable to be reattached by the surgeon due to severe damage caused by the conveyor belt entanglement.
