Investigation Summary
At approximately 11:30 a.m. on June 30, 2008, an employee was managing the warehouse workers, and moving tires from one of the warehouses to the storage compound in preparation for the warehouse to be demolished to make way for a new building. A coworker used a forklift truck to move a pallet of passenger and light truck tires into the storage compound. The employee used a single length of a former extension ladder to move the tires from the pallet to a storage rack. The single length ladder was an old aluminum ladder of unknown manufacture and did not have self-leveling feet. The employee fell about 5 to 6 feet when the ladder slipped out from under her, and she landed on her feet but badly twisted her left ankle, fracturing it. The injury required surgery and she was hospitalized for three days.
