Investigation Summary
On March 5, 2008, Employee #1 and two coworkers were unloading boxes at the back end of a trailer. There were two boxes of mirrors leaning against one wall of the trailer. As the crew removed six other boxes, the large mirror box fell over onto Employee #1, knocking her into the opposite wall of the truck. She hit her head against the wall. She was transported to Sharp Memorial Hospital in San Diego, where she remained for three days with an L4 fracture and a head concussion.
