Empoyee's Finger Is Amputated by Unguarded Chain-Drive

At approximately 8:30 a.m., on April 1, 2008, Employee #1, working for Harbor Packaging Inc., was wearing gloves when he reached into the unguarded area of the chain-drive to remove remnant debris (cardboard fragments) that had collected under the sprocket used for driving the conveyor belt on a machine called a "Flexo-Simon". Employee #1 had a partial amputation of his his middle finger of his right hand. Employee #1 was working as part of a 3-man crew and was stationed at an area identified as the "scrub-belt". The employer manufactures a wide array of packing materials and card-board corrugated containers. Employee #1 was treated and released from Pomerado Hospital for the injury to his finger.The employer reported the incident that resulted in nonserious injuries to DOSH at approximately 2:35 p.m., on April 4, 2008, even though the injury resulted in a partial amputation.
