Investigation Summary
At approximately 9:30 a.m. on September 26, 2007, Employee #1 was unloading a feed truck that was full of corn. The corn dropped into a below grade auger and was transported to an elevator where it was prepared into cattle feed. The auger was unguarded. His foot contacted the rotating auger in an area where a cover plate had become dislodged. He received severe lacerations to his right leg and was hospitalized.
