EMPLOYEE FALLS FROM FORK LIFT AND INJURES BACK

On Friday, November 9, 2007 at approximately 11:30 AM, Employee #1, a 23 year-old warehouseman, working at a records storage warehouse in San Diego, suffered a serious injury when he falls from a pallet on a forklift at the second storage level onto his back. Employee #1's job that day included taking boxes from the first and second level of the storage racks for a records audit by another company. Employee # 1 used a high lift pallet jack to transport the boxes up from the second level. Employee # 1 had put a dozen or so boxes onto the wooden pallet, and was stepping onto the pallet from the second level mezzanine of the storage racks when the pallet broke, Employee # 1's right foot went through the gap between the slats in the pallet, throwing him off balance and causing him to fall to the concrete floor. He was found lying on his back on the floor under the forklift with pieces of the broken pallet around his right leg. Employee #1, was transported to Scripps Mercy Hospital where he underwent treatment for a broken back and was hospitalized for four days.
