Investigation Summary
The employee, a 7 year employee and foreman of the company, had been talking to a driver of a L180E loader articulating vehicle, product serial number L180EV62541#, RT-1158, when he was hit by the claw attachment as it came down on him. The driver had been talking to two other employees to the side of his cab while his left hand was on the steering wheel and his right hand on the control levers. The only way the machine will move or go up or down is by manually touching/steering the controls. The employee died.
