Investigation Summary
On July 29, 2007, Employee #1, a 50 year old truck driver working for a mattress manufacturing shop, picked up a Penske 3-axle conventional sleeper truck, Number 516, without a trailer and drive it to make deliveries. Employee #1 arrived at the jobsite at approximately 4:30 p.m. and pulled the truck up to the gate. Employee #1 used a personal hand vacuum to clean it out and remove some trash bags from inside the cab. At approximately 5:00 p.m. Employee #1 fell from the cab of the truck. Employee #1 didn't know exactly how he fell but he remembers stepping backward with his left foot toward one of the steps on the side of the truck cab. Employee #1 fell backward out of the cab and landed on the asphalt on his left side, fracturing the head of the femur. Employee #1 fractured his hip bone and was hospitalized for two days.
