Investigation Summary
At approximately 6:30 a.m. on June 2, 2007, Employee #1 was working near a poly-film manufacturing machine. The machine had recently been replaced and the rollers were left unguarded. Employee #1's arm was pulled into the in-running nip points of the rollers and he suffered numerous lacerations and fractures to his right arm and hand. Employee #1 was hospitalized.
