Investigation Summary
At approximately 8:20 a.m. on May 1 2007, Employee #1, a carpenter, was constructing a false dormer on the roof of a three story residential building. He was working approximately 30 ft high on scaffolding with no fall protection. He fell over a 12-in. parapet wall landing on a balcony, approximately 11 ft below, and bounced onto scaffolding. He received a cut to his eye and was hospitalized.
