One killed, one injured in fall when scissor lift overturns

At approximately 10:40 a.m. on August 22, 2007, Employees #1 and #2 were using a scissor lift to put up new conduit. The scissor lift was extended to a height of 21 ft when they drove over an open floor drain. The scissor lift tipped over and Employees #1 and #2 fell to the pavement. Both sustained head and neck injuries; Employee #1 was killed and Employee #2 was hospitalized.
