Investigation Summary
At approximately 5:20 p.m. on March 7, 2007, Employees #1 and #2, working for Big Timber Contractors, Inc., were wrapping up work for that day. A forklift operator raised a wooden box constructed of 2-inch by 4-inch boards and 1-inch plywood on three sides. The box measured 10 feet 2 inches long by 3 feet 7 inches wide by 4 feet 4 inches high. A 1-inch natural fiber rope was used to secure the box to the forklift carriage while the box sat on the forks of the forklift. The box was not secured to the forks to prevent lateral movement. The box was elevated up to an opening at the second floor of the building, approximately 10 feet 5 inches off of the ground. Employees #1 and #2 loaded their gear into the box and climbed through the wall opening guardrail into the box, one at a time. While getting into the box, the box became unstable and started sliding off of the forks. Two other coworkers attempted to reach out of the building to keep the box from sliding off of the forklift. Employee #1 fell from the box and landed head first on the ground. Employee #2 landed on his feet and the wooden box fell off the forks and struck him. It then landed on top of Employee #1's head. Employee #1 was killed and Employee #2 suffered unspecified injuries that did not require hospitalization.
