Investigation Summary
Employee #1 was using a Whirlwind cut-off saw, Model Number 212L, Serial Number 1741484L, to cut pieces of wood to make pallets, when his hand came into contact with the moving saw blade. His hand was amputated when the saw cut through his palm. He was hospitalized with his injury. Further investigation revealed that the blade on the saw was designed to retract below the cutting table when not in use and a barrier guard was located above the blade. The saw was operated using a foot pedal, and when cycled, the guard lowered onto the piece being cut as the blade thrust upward through the table opening to make the cut. In this case, the guard on the saw did not prevent the employee from contacting the blade.
