Employee's Finger Is Amputated by Ribbon Mixer

At approximately 12:30 a.m. on November 26, 2008, Employee #1 was performing sanitation duties. As Employee #1 was cleaning an activated ribbon mixer with a hose, she reached into the mixer. Employee #1's right index finger was amputated, and her middle finger was severely injured. She was hospitalized. Employee #1 had not deenergized the mixer prior to sanitation.
