Employee Crushed While Moving Machinery

On September 14, 2007 an employee and two coworkers were moving a piece of machinery within the employer's facility. The machinery being moved was a National Machinery Company Four Station, Three Die Progressive Header (serial number 28176), used to form various pieces of metal hardware. According to the employer the unit weighed approximately 25 tons. To move this unit the employees placed one end of it on two sets of steel rollers, and supported the other end with a Clark Type LP forklift (serial number GPX71OU 0147 9050FC). The forklift was also used to push the Progressive Header on the rollers. At approximately 1:00 p.m. Coworker #1 was driving the Clark forklift pushing the Progressive Header into place along the south wall of the Nedschroeft Room. The forklift was positioned on the west end of the machine. Coworker #2 was standing at the east end of the machine observing the move, and Employee #1 was standing on the south side of the machine also observing the move. As the progressive header was being pushed it suddenly started sliding off the forks of the forklift. Employee #1 was standing between the machine and the south wall as it slid, and was crushed between the Progressive Header and the wall. CPR was administered by a first responder, but the employee died on scene from crushing injuries to the head and chest. Coworkers #1 and #2 both said that Employee #1 was not visible to them prior to the accident.
