Investigation Summary
On August 1, 2007, Employee #1 and Employee #2 were performing preventative maintenance on a Crown stand-up stock picker (Model Number 30SP42TT-S). Employee #1 was a technician for a forklift maintenance contractor, and Employee #2 was an employee of Tiziani Golf Car Corporation. This preventative maintenance work was being done on an inclined, paved surface between the Upper Warehouse and Lower Warehouse at Tiziani's facility. Employee #1 asked Employee #2 to operate the controls of the Crown stock picker to raise the mast so that Employee #1 could apply lubricant to the mast channels. On this particular model of Crown stock picker, the operator controls get elevated along with the lifting forks. As Employee #2 was elevating himself on the control platform, the stock picker started rolling toward Employee #1, who moved out of the way as the stock picker continued to roll. After rolling approximately 10 to 15 feet, the Employee #2 turned the steering wheel of the stock picker. The control platform was elevated to a height of approximately 17 feet. As the stock picker turned, it tipped over. Employee #2 jumped from the control platform as the stock picker was tipping and landed on the pavement below. Contact with the pavement resulted in abrasions, lacerations, and fractures, and he was hospitalized.
