Investigation Summary

Investigation Nr: 201203551
Event: 05/31/2007
Employee Is Injured in Fall from Step Ladder

At approximately 5:00 p.m. on May 31, 2007, an employee was assisting a coworker in the removal and replacement of the second story siding. It was the first day of work for the employee. The employee was using a 12-ft step ladder and the coworker was using an extension ladder. At about 5:00 p.m., the employee and coworker finished installing a 4 by 8 by 0.5-in. sheet of siding. The employee then descended the step ladder repositioned it, picked up another 4 by 8 sheet of siding from the concrete parking lot and moved to the step ladder with it. He was in the process of raising it up into position, while standing on the third or fourth step of the step ladder. Something occurred, and the panel slid downward out of position. The employee tried to stop and catch the panel. At that time, the step ladder tipped over and the employee fell to the concrete parking lot, fracturing the right side of his skull and injuring his ankle. The employee was knocked unconscious. The emergency medical service was called and responded. The accident site was only 10 blocks from the hospital. The employee was treated for his injuries and released the next day.
