Investigation Summary

At approximately 6:45 a.m. on January 8, 2007, an employee, a temporary service employee, was working as a security guard. He was struck by steel pipe that was being relocated from one area to another by a fork truck. Apparently, while the operator was backing and turning with the load of four pipe joints the end section of the 16 inch diameter by 42 feet long pipe, which extended approximately 15 feet beyond the forks, struck the employee in the chest as he was sitting in a chair by his car. The employee's car was parked in front of a pile of pipe outside the parking area for the guard shack. The employee was hospitalized and died from his injuries on January 22.
