Investigation Summary
On January 11, 2007, Employee #1 was installing siding on the exterior of a single family home undergoing construction. The building was approximately 4,500 square feet. Employee #1 and coworkers erected a pump jack scaffold and scaffold platforms out of extension ladders and boards that rested on the rungs of the extension. The scaffold platforms were not secured to prevent displacement. During the installation of the siding, Employee #1 and his coworkers were required to work from the roof of the homes porch, which had a slope of 6 to 12 and was covered with frost. While this portion of work was being performed, Employee #1 and a coworker staged the needed materials at the top of the erected scaffolding. The coworker carried the packages of siding, two at a time, up the extension ladder used to access the roof and handed them to Employee #1, who then carried them to the to of the fabricated scaffolding that measured 28 feet high. As the staged materials increased in number, they became displaced, causing Employee #1 to fall approximately 14.5 feet to the ground. Emergency services were contacted, and Employee #1 was transported by ambulance to Suburban Hospital in Bethesda, MD, where he was treated for abrasions and contusions received.
