Investigation Summary
At approximately 9:00 a.m. on May 24, 2007, an operator was operating a "clark" gcx 25 propane rider forklift that was returning from dumping garbage from a tote into a dumpster. The operator was attempting to drive around recent asphalt repairs. The forlift's left rear wheel dropped off the pavement, and the forklift became stuck. An employee and four coworkers came to assist in freeing the forklift by getting inside the tote on the forks and standing on the forklift frame by acting as counterweights on the forklift. At this time, the tote broke and flipped off the forks throwing the employee and coworkers onto the pavement. The employee struck his head on the pavement and died two days later from his injury.
