Investigation Summary
On February 14, 2007, an employee was installing a sprinkler hanger while standing on a 20 foot high wine tank. He was not tied off with a fall protection. He fell to the concrete floor, fracturing a vertebra and his wrist. He was hospitalized and treated for his injuries.
