Investigation Summary
At approximately 3:15 a.m. on February 9, 2007, Employee #1 was working for a manufacturer of semiconductors and related solid-state devices. He was attempting to remove a piece of trash from inside a running dry film laminator. While his hand was inside the machine, it cycled, catching his fingers between the hot rolls and the film guide of the machine. The ring and middle fingers on his right hand were crushed and burned. Paramedics were summoned to the site, and Employee #1 was transported to Valley Medical Center in San Jose, CA. He was hospitalized.
