Investigation Summary
On March 22, 2007, Employee #1 was removing insulation on a flat roof. The roof contained metal pans into which light weight concrete was poured. He stepped on the light weight concrete and the metal pan gave way, probably due to rust. Employee #1 fell and was killed.
