Investigation Summary
On October 18, 2006, Employees #1 and #2 were prepping windows for installation. Windows were stored in a storage trailer on the site. The Employees would take out one window at a time and prepare it by drilling holes. They would then put window back in trailer and take out another one. The windows were stacked very close because trailer was very full, when Employee #1 tried to take out another window; it created a domino effect causing windows to fall over on both employees. Employee #1 was crushed to death, and Employee #2 was hospitalized for chest injuries.
