Investigation Summary
At approximately 10:00 a.m. on May 6, 2009, Employee #1 was working alone installing a counterweight (solid steel) on the back of a Caterpillar Forklift (Serial Number T-80C-54Y00562). The counterweight was elevated on the 1989 Taylor Forklift (Serial Number SH5-23048). The employee needed help, so a coworker jumped on the Caterpillar Forklift and the employee was between the two forklifts and the elevated counterweight. The employee's right foot was on the bottom of the counterweight and he was attempting to put the counterweight in the saddle on the Caterpillar Forklift. For whatever reason, the coworker started backing the Caterpillar Forklift without first checking to see if employee was clear of it. The employee was pinned between the two forklifts and sustained multiple injuries that included a dislocated right hip, right knee and lower back problems. Employee #1 was hospitalized.
