Investigation Summary
At approximately 11:20 p.m. on April 28, 2009, Employee #1, a press operator, was retrieving a five-gallon plastic bucket from an outside storage shed located in the western part of the manufacturing facility. The storage shed held solvent wastes and used hydraulic oil. Waste acetone was contained within the bucket. As he picked up the bucket, a sudden spark occurred that ignited the acetone and produced a flash fire. The fire ignited his clothing and caused second and third degree burns to his left side, including his arm, upper body and leg. A supervisor initially transported him to the Central Occupational Medical Center for treatment, but he was referred to San Antonio Community Hospital, Upland, CA. He was ultimately transported by ambulance to the Arrowhead Regional Medical Center, Colton, CA. At the medical center, he received surgical and burn treatment, and was hospitalized for 10 days.
