Investigation Summary
At about 12:05 a.m. on March 27, 2009, Employee #1 was operating a "Yale stand-in type reach fork lift (Model Number NR045EANS36TE131, designated as Number 5) to move empty pallets within a paint distribution center. While maneuvering the fork lift in a backwards direction, he lost control and the forklift struck a vertical rack member. Employee #1's left foot became pinned between the lift bottom frame and the rack. He was hospitalized at Riverside Regional Medical Center for fractures.
