Investigation Summary
At approximately 2:00 p.m. on April 23, 2009, Employee #1 was using a Werner 24-ft portable aluminum extension ladder to install a sheet metal roof. The ladder was leaning against the building with the top rail below the eave. The ladder was not tied, blocked, or otherwise secured to prevent it from being displaced. The lowest edge of the eave was measured to be 12.75 feet off the dirt grade. Employee #1 was standing on the ladder, approximately 8 feet above the ground. The unsecured ladder shifted to the left, fell to the ground on its side and Employee #1 landed on top of it. He sustained multiple, unspecified fractures and was transported to Inland Valley Regional Medical Center, where he was hospitalized for 11 days.
