Employee Makes Contact with Auger, Amputates Arm

At approximately 2:30 p.m. on April 6, 2009, Employee #1 was at the end of his shift when a customer came in and wanted crushed ice. Employee #1 activated a machine that crushes the ice. The ice was not coming out fast enough so Employee #1 opened the auger trough to retrieve more crush ice. The auger was not "on"; however, as Employee #1 was retrieving more crush ice, the auger came "on" catching Employee #1's left arm. Employee #1's arm was amputated, and he was hospitalized for treatment.
