Investigation Summary
On November 6th 2008 Employee #1 received head injuries when he fell off of the back of a golf cart at about 3:30 PM as he and two other employees were returning to their residences within the mobile home park. There were only two available seats provided on the ER1's Textron EZ-Go, TXT-PDS model golf cart, SN: 1283345, so Employee #1 rode on the back by standing in the bag well at the back of the golf cart and holding the vertical supports for the sunscreen top. His left foot slipped off the trailer hitch ball and fell from the cart and struck his head on the pavement. Employee #1 was transported to Arrowhead Regional Medical Center where he was admitted and received medical treatment until his release two days later on 8 November 2008.
