Investigation Summary
At approximately 4:00 p.m. on July 28, 2008, Employee #1 fractured both wrists when he fell from a palm tree. Employee #1 was hospitalized for ten days at Arrowhead Regional Medical Center, Colton, Ca. Employee #1 was approximately 15 ft high trimming a palm tree. Employee #1 was wearing the appropriate safety gear. Employee #1 thought that the belt that he was wearing broke, however, when investigated, the belt had not broken and was still intact. It is possible that the nylon rope used to go around the palm tree got caught in the double action snap hook and caused it to open.
